NAME CHANGE CHECKLIST
Here's a list of places that you should remember to contact if you decide to change your name after the wedding:
When changing your last name, you will first need to obtain a certified copy of your marriage certificate from the County Recorder. Here is the Aplication Form. Some agencies will require a copy of the certified license. Remember that the marriage certificate will not be automatically sent to you. You must request it.
Once you have the certificate, you must take it to the Social Security office to change your name with them. After that, you need to change it at the DMV. Once you receive the new social security card and dricer license in the mail, you may procees with contacting all the businesses and government agencies that still have your maiden name.
